Cassiopeia "Cassie" Sullivan's story begins just after the 4th Wave. She describes what has happened over the course of the last year or two. In the 1st Wave, the alien ship appears in the sky, and all electricity is destroyed by the use of EMPs (Electromagnetic Pulses). The 2nd Wave included a bar dropped from outer space that destroyed every coastline in the world. The 3rd Wave brought with it a terrible bird-borne virus similar to Ebola, which is known by many names including The Red Death. The victims of The Red Death have a 1-in-10 chance of surviving, and due to this disease nearly all 7 billion people are wiped out. In the 4th Wave, it is discovered that certain people have an alien being residing inside their head. These people might as well be dead, because the aliens are controlling what they do. They are known as Silencers, and because they look as normal as anyone else, Cassie learns to follow one simple rule; trust no one.
Now the 5th Wave is about to begin. Sammy, her little brother, has been taken away to a military base, and out of a promise she made to him, Cassie sets out on a journey to find him. Her search comes to a halt when she is shot in the leg by a Silencer. She falls to the ground, severely injured, and snow begins to fall. She is then rescued by a farm boy named Evan Walker. Evan helps her back to health, and Cassie tells him that she promised her brother Sammy that she would find him, so she must go on without Evan, even with an injured leg. Evan says that he will go with her, despite the risk.
But Cassie isn't so sure. She suspects that Evan might be a Silencer. Cassie now has a choice to make. Should she go against her rule and trust Evan blindly? And will she survive the agony of the 5th Wave?
